ABSTRACT

Folliculosebaceous cystic hamartoma is a rare, benign condition. The histological criteria of folliculosebaceous cystic hamartoma have been well established by Kimura et al. We report two cases of folliculosebaceous cystic hamartoma which showed typical histological findings. As far as we know, it has not yet been reported in Korean dermatological literature. The first case is a 23-year-old man who had a pedunculated nodule on the scalp. The second one is a 46-year-old man who had a bean sized pedunculated nodule on the left cheek.

ABSTRACT

BACKGROUND: The nevus of Ota is a benign dermal melanocytic lesion that was previously proved difficult to treat. Recently, the Q-switched ruby laser and Q-switched Nd: YAG laser have been reported to by, successful in the treatment of benign pigmented lesions and tattos. OBJECTIVE: Our study evaluates the treatment of 60 patients with nevus of Ota with Q-switched Nd: YAG lase (l064nm). METHOD: Sixty patients with nevus of Ota were treated by the Q-switched Nd: YAG laser with pulse width 10nsec, and energy fluences ranging from 5.8 to 12J/cm and the laser repetition rate was 5 or 10Hz. The response to treatment was assessed by four independent investigators with photographs. RESULTS: 1. Total therapeutic effects of Q-switched Nd: YAG laser showed excellent result in 23.3%, good in 45%, fair in 18,3%, and poor in 13.3%. 2. Therapeutic effects by distribution proposed by Tanino showed excellent or good results in 84.6% at type Ia, 78.6% at, type Ib, 66.7% at type II, 60% at type III and 45.5% at type N. 3. Therapeutic effects by the number of treatments showed excellent or good results in 22.2% of patients after one treatment, 72.7% after two treatments, 93.3% after three treatments and 100% after four treatment. 4. Therapeutic effects by energy showed excellent or good results in 9.2% at energy range of 10 to 12J/arl and 66.6%, at energy range of 5.8-9.5J/cm. 5. Complications included transient hyperpigmentation (6.7% ), secondary bacterial infection (1.7 %), hypopigmentation (1.7%) and atrophic scar (1.7%). However these subsided spontaneously in a year. 6. The recurrence rate was 2.4%. CONCLUSION: The Q-switched Nd: YAG laser is a very effective mode of treatment for nevus of Ota which has provent unresponsive to other therapeutic modalities.

ABSTRACT

Angiofibroma is a disfiguring facial deformity that constitutes pait of the multiple system involvement in tuberous sclerosis. We describe eight affected patients who have been treated with:arbon dioxide laserabration and subsequent copper vapor later treatment for residual angiomatous component, which resulted in conspicious cosmetic improvement without scarring or recurrence. This report documents that the treatment with carbon dioxide later and subsequent copper vapor laser is an efficient and safe remedy for treating multiple facial angioibromas.

ABSTRACT

Nevus flammeus is a benign but cosmetically devascating congenital inalformation involving mature capillaries. The copper vapor laser is a therapeutic device which had been newly applied to this. Yellow copper vapor light is absorbed by the intraluminal oxyhemoglobulin mo ecules, leading to the destruction of blood vessels. This study was undertaken to evaluate the histobigic findings in a patient with nevus flammeus according to the sequential time after copper vapor laser therapy. The results were as followed, One hour after treatment, nonspecific damages to the epidermis and upper part of the dermis were observed. The specific damage to the vessels which showed thrombi formations and partial disruption of the vessel walls was prominent. One cay after treatment, degeneration of the epidermis and sepaiation of the dermoepidermal junction were apparent. Three days after treatment, extravasations of a small amount of erythrocytes and inflanimatory cell infiltrations were present, especially around the vessels. Seven days after treatment, the capillaries showed a deposition of fiorinoid material around themselves. Partial disappearance of the vessel walls was seen. There was hisi ologic evidence of the replacement of abnormal ectatic vessels by normal appearing vessels with small uminal diameters, surrounded by endothelial cells.

ABSTRACT

At twenty two year old female with nevus comedonicus and an increased tendency to form keloids is described. Although example of successful treatment of nevus comedonicus are readily found in the literature none propose reasonable therapeutic models for keloid formers. As our patient clearly demonstrated this tendency. We initated a t,rial with the carbon dioxide laser due to it's ability to cause less thermal damage. Subsequent to completion of this t,rial, the lesion has imprcived with the acceptable cosmetic results and less comedogenic activity.

ABSTRACT

Fifty-two Korean patients with Behcet's syndome were typed for HLA antigens. 52 apparently healthy Korean subjects were used as controls; 42 for HLA-A, B, C and all 52 for HLA-DR typing. HLA-B5 and DRw8 presented significantly high frequencies in all patiens. According to Shimizu's classification, HLA-B5 and DRw8 were significantly increased in the complete type; B5 in the incomplete type; DR3 in the suspected-possible type. According to Lehner's classification, HLA-DR3 was significantly increased in the neurological type; B5 in the ocular type; B5 in the ocular type. We confirmed the association of HLA-B5 with the severity of Beh et's syndrome. A relation might exist between DRw8, DR3 and Behcet's syndrome.

ABSTRACT

Herpes zoster is known to recur only rarely. However reports on the recurrence rates by various authors ranges from 0.l% to 8%. We have experienced 2 cases of recurrent herpes zoster in a 63-year-old female and a 62-year-old male who presented with typical clinical features and previous history of herpes zoster. They were suffering from breast cancer and Hodgkin's disease, respectively. By employing negative and positive staining methods of electron microscopy, typical herpes viral particles were demonstrated from the vesicular fluid.

ABSTRACT

Various immunologic investigations of 159 patients with Behqets syndrome undertaken included; the DNCB sensitization test, total T-cells and T-cell subsets and the lymphocyte transformation test using PHA. The percentage of positive esponsiveness to DNCB decreased in the order of possible (65%), suspected (60%), incomplete (37%) and complete type (37%), The number of patients with impaired LTT was larger in the group of patients with DNCB( + ) responsiveness (seventeen of 32) than in DNCB(+) group (six of 25), There was significant impairment of cell mediated immunity in Behcets syndrome compared to normal subjects, when analyzed by the impaired LTT and decreased proportions of helper T-cells.

ABSTRACT

Lichen aureus is a rare disorder classified as a pigmented purpuric dermatosis. This case report is of a 46-year-old female in whom a coin sized, gold tinted, erythematous maculopapule developed on the right lower quadrant of the abdomen l0 years ago which showed characteristic histological findings of lichen aureus consisting of lymphohistiocytic infiltration in the upper dermis and extravasation of red blood cells.

ABSTRACT

Woringer-Kolopp disease, so called pagetoid reticulosis, is most likely represent a chronic localized from of reticulosis. A 24-year old male patient visited with asymptomatic slowly enlarging, scaly single patch on his left thigh for 10 years duration. Histopathologic studies were taken with H-E stain, D-PAS, transmission electronmicroscopy and monoclonal antibody(OK6) tissue stain. Infiltration of atypical cells showed in the lower portion of epidermis like pagetoid patterned, none in dermis. It seems to be that the cell in this case probably originated from the stimuIated T-lymphocyte.

ABSTRACT

This study was undertaken to evaluate the efficacy and safety of orally administered ketoconazole in the treatment of onychomycosis. Thirteen patients with onychomycoiis were selected for this study. For evaluation of ketoconazole efficacy twelve patients received a 200 mg tablet of ketoconazole daily and one patient received a. 10() mg tablet of ketoconazole daily. All patients were studied monthly clinically and mycologically by KOH mount and culture on Sabourauds medium. The results were as follows: 1. Seven(53.8%) of thirteen cases achieved remission, marked improvement occurred in three cases(23.1%) and moderate improvement occurred in three cases (23. 1 %). 2. The mean duration of remission or clinical improvement was 5 months for all lesions and toenail lesions(5. 4 months) took longer to heal than finger nail lesions (4 months). 3. One hundred percent of patient who previously failed to respond to griseofulvin were cured or clinically improved by ketoconazole. 4. Two patients cornplained of indigestion and one of headache. In our study, ketoconazole was highly effective in the treatment of onychomycosis.

ABSTRACT

Kaposi's sarcorna is a tumor-like condition which characterized by a mixed proliferation of fibroblastic components and vascular structures. The authors report a case of nodular type of Kaposis sarcoma in 54 year-old male who had an eroded hemorrhagic nodule and few papular lesions. These lesions involved only both hands and feet without systemic involvement.
